本表格所收集的個人資料將會絕對保密，有關資料只用作確認你參加「賽馬會幼童健齒計劃 」(本計劃) 之網上自學課程。除非獲得資料當事人同意或在法律強制下，否則我方不會向第三方披露參加者所提供的任何個人資料。所得的個人資料將會於本計劃完結後三個月內銷毀，或於閣下提出要求時立即刪除。

 

根據《個人資料(私隱)條例》，所屬資料的當事人有權查閱及更正其個人資料。如欲查閱或更改閣下於報名表上所提交的個人資料，請電郵至 jccohp@hku.hk。

Personal data collected in this form will be kept strictly confidential. The information collected will be used for confirming your enrolment in the Online Self-Learning Course provided by "Jockey Club Children Oral Health Project" (the project) only. We do not generally disclose your personal information to third parties without your consent unless required by applicable law. The collected data will be purged within three months after the completion of the project or will be immediately omitted upon request.

 

Under the Personal Data (Privacy) Ordinance, you have the right to request access to and correction of information held by us about you. If you wish to access or correct your personal data, please contact us via email jccohp@hku.hk.
