The personal data collected in this form will be kept strictly confidential and will only be used to confirm your participation in the online self-study course of the "Jockey Club Children's Dental Health Project" (the project). We will not disclose any personal data provided by participants to third parties unless we have the consent of the data subject or are compelled by law. The personal data obtained will be destroyed within three months after the completion of the project, or deleted immediately upon your request.
